Community provider meeting focused on developing collaborative team care plans to support vulnerable adults aged 18-59 experiencing chronic crisis due to health and behavioral health issues. Community providers also participate in a Division of Child and Family Services MDT, and an Elder Protective Services MDT.

Multidisciplinary group of community stakeholders, including treatment providers, hospital partners, emergency services/fire, law enforcement, social services, and peer and family advocates, that meets monthly to identify community behavioral health gaps and needs, and develop solutions to develop and enhance local behavioral health resources.
